COVID-19 Pre-Screening & Clinic Info/Procedures:

COVID-19 Appointment Pre-Screening
Prior to any appointment, patients will be pre-screened using the following questions:
1. Did you receive your final (or 2nd) vaccination dose more than 14 days ago?
(A fully immunized individual is defined as any individual >14 days after receiving their second dose of a two-dose COVID-19 vaccine series or their first dose of a one-dose COVID-19 vaccine series (i.e .Johnson and Johnson).
2. Do you have any of the following symptoms?
Fever and/or chills
New onset of cough or worsening chronic cough
Shortness of breath
Decrease or loss of sense of taste or smell
Sore throat
Runny or stuffy / congested nose
Headache
Nausea / vomiting / diarrhea
Unexplained fatigue / lethargy / malaise / muscle aches (myalgia)
Feeling confused or unsure of where you are
Losing consciousness
3. Have you tested positive for COVID-19 in the past 10 days or have you been told you should be isolating?
Additional questions to be answered if you are NOT fully immunized (i.e. you answered ‘No’ to question #1):
4. Have you travelled outside of Canada in the past 14 days?
5. Have you had close contact with a confirmed case of COVID-19 without wearing appropriate PPE?
***If you have answered YES to questions 2, 3, 4, or 5:***
(1) speak with your chiropractor and do NOT attend in person at our office for at least 14 days;
(2) get tested for COVID-19;
(3) complete the Ontario Government’s self-assessment; and
(4) contact an appropriate medical authority (medical doctor, public health agency, Telehealth Ontario, local assessment centre).

COVID-19 Clinic Policies & Patient Information
In order to help keep everyone safe, you’ll notice a few physical and procedural changes around the office. We have taken additional steps for cleaning, disinfection, and physical distancing, have made changes to appointment times, practitioner schedules, and spacing, have implemented the use of personal protective equipment for all staff and practitioners. Your health and safety is our top priority.
We kindly ask all patients and clinic visitors to do the following:
Wear a mask that covers your face, mouth, and chin, to your visit. If you don’t have one, we will provide one for you at a cost of $2.
Although we love seeing your family and friends, please come to your appointment alone. This helps us to limit the number of people in our office so we can all adhere to physical distancing requirements. Patients who are minors may bring one (1) guardian, who must also compete the COVID-19 appointment pre-screening prior to arrival. If you are not a minor, and need physical assistance, please let us know.
Comply with COVID-19 pre-visit screening. There will be a mandatory screening conducted on the day of, prior to your appointment, and again once you’ve arrived at the clinic. The questions are posted on this site (above) — so you can review them at any time.
Reschedule your visit if you’re feeling unwell. Our clinic does have a 24 hour cancellation policy, but with the current pandemic, cancellation fees will be waived for any cancelled appointments due to illness. The more notice you can give us though, the better. Have questions or concerns? Please ask!
Thoroughly sanitize your hands immediately upon arrival, after touching common surfaces and upon exiting our clinic. Hand sanitizer is readily available throughout our office.
Follow signs and directions in the clinic regarding room capacity, symptom screening, and physical distancing.
Arrive on time, at your scheduled appointment time. This helps us, helps you, and helps the rest of our patients. Please follow signs in our space, in order to adhere to physical distancing while waiting for your appointment. If you wish to remain in your car until your appointment, please phone us upon your arrival and we will let you know when we are ready for you.

We encourage everyone to utilize contactless payment methods (Visa / debit / MasterCard / email money transfer) and we will be happy to email you your paid invoice. These measures will reduce the amount of paper transactions for the time being.
